Excellus dental plans are only available for businesses and require a minimum of two employees to enroll. Guardian and MVP PPO dental plans fulfill the pediatric dental rider mandate which will allow medical to be purchased without the pediatric dental rider included.
Below are dental benefits available for Chamber employees only. This benefit will be billed quarterly for all plans. For more information on billing, click here.
If you are not making any changes to any of your benefit plans, you do not need to complete any forms.
If we do not hear from you we will assume no changes to your plan offerings.
*There are no new hire waiting periods on coverage for dental or vision if members enroll within their new hire period. Bond needs to be notified of new hire elections within 30 days of the member’s date of hire or the member would be subject to late entrant penalties for dental, and a lock-out until Open Enrollment for vision. (The signature date on the application will be the effective date if it doesn’t match the listed DOH.)
You need to recertify dependent student status for any children reaching age 19 or over annually or their coverage will terminate. In addition to completing the Student Certification form, You’ll also need to send over ONE of the following:

There are two dental options available from Excellus BlueCross BlueShield. Dental Blue Options – Voluntary Option 4 and Dental Blue Options – Contributory Option 5.
Voluntary Option 4 is 100% employee paid – these plans can have no portion of the cost paid by the employer.
Contributory Option 5 requires that the employer pay at least 25% of the cost of the plan.
